Education Session Request
Thank you for your interest in requesting education from our service.
This form is for organisations seeking education, training, or information sessions delivered by our clinical team. Please complete all relevant sections to help us assess your request fairly and efficiently.
Please note:
- We prioritise education that supports our strategic focus areas (e.g. palliative care in community and aged care settings).
- Requests are reviewed regularly and subject to staff availability.
- Completing this form does not guarantee approval, but all requests will be acknowledged.
- Please allow at least 4 weeks’ notice before your preferred date to assist with scheduling.
- You can request a session up to 12 months in advance.
Once submitted, your request will be reviewed, and a member of our team will contact you to confirm next steps or request more information.
